Ear pain is a common symptom that can be caused by a variety of conditions, including infections, inflammations, injuries, or diseases affecting the internal or external structures of the ear. In medicine, ear pain is classified as "otalgia". It is important to consider that it can be either primary or secondary, associated with diseases not related to the ear, such as dental or throat diseases.
Ear pain classification
International medical standards classify ear pain into different categories depending on the location, duration, and intensity of the pain syndrome:
1. Primary otalgia - pain caused by pathologies of the ear itself (e.g. middle ear infections).
2. Secondary otalgia - pain that comes from structures not directly related to the ear (e.g. toothache, sore throat, cervical vertebrae diseases).
Causes of ear pain
Infectious diseases
Acute otitis media (middle ear infection) is one of the most common causes of pain in children and adults. The infection can be bacterial or viral.
Otitis externa (otitis externa) is an infection of the external auditory canal, often caused by bacterial infections.
Mastoiditis is an inflammation of the bone tissue in the mastoid process. This is a complication of otitis media that requires intensive treatment with antibiotics.
Traumatic injuries
Trauma to the outer ear or eardrum (for example, due to foreign bodies or strong pressure) can cause acute pain, bleeding and/or hearing loss.
Diseases of the teeth and throat
Toothache (for example, inflammation of the dental nerve) can reflect ear pain, which is a typical example of secondary otalgia.
Diseases of the nasopharynx - inflammatory diseases such as pharyngitis or tonsillitis can also cause ear pain through reflex spread of pain along the nerve pathways.
Problems with the maxillofacial apparatus
Dysfunction of the temporomandibular joint (TMJ) can cause ear pain, which is often accompanied by clicking or crunching when moving the jaw.
Hearing disorders
Earwax or foreign bodies in the external auditory canal can cause ear pain, a feeling of congestion and hearing loss.
Diagnosis of ear pain
Diagnosis of ear pain is based on a multi-step approach:
Collection of anamnesis
Anamnesis helps to establish the exact cause of the pain syndrome.
Duration and type of pain (sharp, dull, throbbing, etc.).
Symptoms associated with pain (discharge, hearing loss, headache).
Examination of the ear canal using an otoscope allows the doctor to see the condition of the eardrum, the presence of inflammation or fluid in the middle ear.
Hearing tests can help identify problems with hearing (eg, audiometry).
Additional examinations
CT or MRI may be ordered if complex diseases such as mastoiditis or tumors are suspected.
Microbiological examination (eg, swab from the ear canal) can help determine the presence of infections and sensitivity to antibiotics.
Ear Pain Treatment
Treatment of ear infections
Antibiotics: The mainstay of treatment for ear infections is the administration of antibiotics, especially in the case of a bacterial infection.
Anti-inflammatory drugs: Non-steroidal anti-inflammatory drugs are used to relieve inflammation and pain.
Local treatment: Ear drops can be used for otitis externa.
Surgical treatment
In some cases, surgery may be required, such as in the case of a perforated eardrum or purulent mastoiditis.
Treatment of ear injuries
In case of injuries to the outer ear or eardrum, rest is recommended, in some cases antibiotics and surgery.
Pain relief and supportive care
Painkillers are used to relieve pain
Treatment of diseases not related to the ear
In the presence of secondary pain, caused, for example, by a dental infection or TMJ problems, treatment is aimed at eliminating the underlying cause.
Prevention and recommendations
Protection against infection
Regular ear hygiene, use of protective equipment when swimming.
Prevention of colds, timely treatment of inflammation in the nasopharynx and oral cavity.
Prevention of injuries
Avoid mechanical impact on the ear, do not use sharp objects to clean the ears.
Timely referral to a specialist
If ear pain occurs, it is important to consult a doctor for an accurate diagnosis and appropriate treatment.
